Cifran CTH is a fixed-dose combination of ciprofloxacin and tinidazole used to treat certain bacterial and protozoal infections, particularly in the gastrointestinal and genitourinary tracts. It is commonly prescribed for infectious diarrhea or dysentery suspected to be due to mixed pathogens, some pelvic and intra-abdominal infections, and other polymicrobial infections where both aerobic and anaerobic organisms may be involved. The exact indication for an individual patient should be determined by a qualified doctor based on symptoms, examination, and, when available, laboratory results.
Cifran CTH contains ciprofloxacin, a fluoroquinolone antibiotic, and tinidazole, a nitroimidazole antiprotozoal and antibacterial agent. Ciprofloxacin inhibits bacterial enzymes DNA gyrase and topoisomerase IV, which are essential for DNA replication and repair, leading to bacterial cell death. Tinidazole enters susceptible anaerobic bacteria and protozoa and forms reactive intermediates that damage DNA, resulting in cell death. The combination provides broad coverage against certain gram-negative bacteria, some gram-positive bacteria, anaerobes, and protozoa, making it useful for mixed infections.
Cifran CTH should be taken exactly as prescribed by your doctor. The tablets are usually swallowed whole with a glass of water and can often be taken with food to reduce stomach upset, unless your doctor advises otherwise. Do not crush or chew the tablets unless specifically instructed. Take the medicine at evenly spaced intervals to maintain consistent levels in your body. Do not change the dose, dosing interval, or duration of treatment on your own, and do not share your medicine with others.
If you miss a dose of Cifran CTH, take it as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and take the next one at the regular time. Do not take two doses together to make up for a missed dose, as this may increase the risk of side effects without improving effectiveness. If you frequently forget doses, speak with your healthcare provider about strategies to improve adherence or whether an alternative regimen is more suitable.
Common side effects of Cifran CTH include nausea, vomiting, abdominal pain, diarrhea, indigestion, metallic or altered taste, loss of appetite, headache, dizziness, and general weakness or fatigue. Some people may also experience mild skin rash or itching. These effects are often temporary and may improve as your body adjusts to the medicine. However, if side effects are severe, persistent, or worsening, you should contact your doctor promptly for assessment and possible adjustment of therapy.
Serious side effects, although less common, require urgent medical attention. These include tendon pain, swelling, or sudden tendon rupture (especially in the Achilles tendon), severe muscle or joint pain, new or worsening nerve symptoms such as numbness, tingling, burning, or weakness, seizures, confusion, hallucinations, severe mood changes, irregular heartbeat or palpitations, severe or persistent diarrhea (which may indicate Clostridioides difficile-associated diarrhea), yellowing of the skin or eyes, dark urine, severe skin reactions with blistering or peeling, and signs of a severe allergic reaction such as difficulty breathing, swelling of the face or throat, or widespread rash. If any of these occur, stop the medicine and seek immediate medical care.
Alcohol should be strictly avoided while taking Cifran CTH and for a period after completing therapy, as advised by your doctor. Tinidazole can cause a disulfiram-like reaction with alcohol, leading to flushing, throbbing headache, nausea, vomiting, abdominal cramps, and a rapid heartbeat. Even small amounts of alcohol, including in some cough syrups, mouthwashes, or fermented foods, may trigger this reaction. To reduce the risk of serious discomfort and complications, abstain from alcohol during treatment and follow your doctor’s guidance on when it is safe to resume drinking.
Use of Cifran CTH during pregnancy is generally not preferred and is considered only when the expected benefit to the mother clearly outweighs potential risks to the fetus. Fluoroquinolones like ciprofloxacin have raised concerns in animal studies regarding effects on developing cartilage, and tinidazole is usually avoided, especially in the first trimester. If you are pregnant, think you might be pregnant, or are planning to conceive, you must inform your doctor before starting Cifran CTH. Your doctor may choose an alternative antibiotic with a more established safety profile in pregnancy whenever possible.
Both ciprofloxacin and tinidazole can pass into breast milk. Because of potential adverse effects on the nursing infant, including possible effects on cartilage development from ciprofloxacin and theoretical mutagenic or carcinogenic concerns with tinidazole, Cifran CTH is usually avoided during breastfeeding unless no suitable alternative exists. If your doctor decides that Cifran CTH is necessary, they may advise you to temporarily interrupt breastfeeding and express and discard breast milk during treatment and for a recommended period after the last dose. Always follow your doctor’s specific advice regarding breastfeeding and this medicine.
Cifran CTH is generally not the first choice in children and adolescents because fluoroquinolones like ciprofloxacin have been associated with effects on growing cartilage and an increased risk of musculoskeletal adverse events. In certain serious infections where no suitable alternatives are available and the benefits outweigh the risks, a specialist may prescribe ciprofloxacin-based regimens to children under close supervision. Tinidazole use in children is also subject to age- and weight-based considerations. Any use of Cifran CTH in pediatric patients must be strictly guided by a pediatrician or infectious disease specialist.
