Because it is used for serious infections and can affect the kidneys and nerves, patients are monitored closely during treatment. Common: kidney effects (reduced kidney function, usually reversible after stopping), tingling or numbness around the mouth and in the hands and feet, dizziness, and injection-site reactions. Nausea and vomiting can also occur. Nervous system: tingling, visual disturbances, slurred speech, and unsteadiness (ataxia), more likely at higher doses. Serious (require immediate medical attention): significant kidney injury, severe allergic reactions, and — rarely — breathing difficulty from muscle weakness (neuromuscular blockade), especially in people with kidney problems or those on certain muscle relaxants. Nebulised colistin can sometimes cause coughing or chest tightness.
It is given by a healthcare professional, usually as a slow injection or drip into a vein, in divided doses (often every 8 hours). For some lung infections it may be given as an inhaled (nebulised) solution. It is not self-administered at home unless specifically arranged and taught by your care team.
The dose is based on your body weight, kidney function, and the severity and type of infection, so it is individualised by your doctor. Kidney function is checked regularly and the dose may be adjusted during treatment.
Colistin can temporarily affect the kidneys, and the effect is dose-related. Regular blood tests let the team catch any changes early and adjust the dose; kidney function usually recovers after treatment ends.
Inform them if you have kidney problems, myasthenia gravis or other muscle conditions, or if you are pregnant or breastfeeding. Also list all your medicines — especially other antibiotics that affect the kidneys (aminoglycosides), diuretics, and muscle relaxants — as these can add to the risk.
Colistin is reserved for serious, multidrug-resistant infections to preserve its effectiveness and because it carries a higher risk of kidney and nerve side effects than many routine antibiotics. It is chosen when the benefits clearly outweigh these risks.
