Hepamerz OA Syrup 100 ml is a medication used to treat liver diseases.
It helps in treating liver diseases such as hepatic encephalopathy and fatty liver.
The dosage is determined by the physician. Usually, it is taken two or three times a day.
Common side effects include nausea, vomiting, and upset stomach.

It is commonly prescribed in conditions such as hepatic encephalopathy and other acute or chronic liver disorders where ammonia detoxification is impaired. By supporting the conversion of ammonia to urea and glutamine, it may help improve neurological symptoms like confusion, drowsiness, and impaired concentration associated with high ammonia.
The active ingredient in Hepamerz syrup, L-ornithine L-aspartate, provides substrates for the urea cycle and for glutamine synthesis. Ornithine acts as a key component of the urea cycle in the liver, promoting the conversion of toxic ammonia into urea, which is excreted by the kidneys. Aspartate contributes to glutamine formation, another pathway for ammonia detoxification. Together, these mechanisms help lower circulating ammonia levels and may alleviate symptoms of hepatic encephalopathy and related cognitive disturbances.
Hepamerz syrup should not be taken without medical advice. It is intended for specific clinical situations, mainly related to liver dysfunction and elevated ammonia levels. Self-medicating for general fatigue, indigestion, or as a so-called liver “tonic” is not appropriate and may delay proper diagnosis of an underlying condition. A doctor should evaluate your liver function, overall health, and current medications before deciding whether Hepamerz syrup is suitable for you.
Common side effects reported with L-ornithine L-aspartate preparations such as Hepamerz syrup include gastrointestinal symptoms like nausea, vomiting, abdominal discomfort, and diarrhea. These are often mild and may be related to higher doses. Less commonly, patients may experience bloating, a feeling of fullness, or altered taste. In such cases, the doctor may avoid use, adjust the dose, or monitor kidney function and clinical status closely. Patients with known kidney disease should never start Hepamerz syrup without explicit medical supervision.
Data on the use of L-ornithine L-aspartate in pregnancy are limited. Hepamerz syrup should only be used during pregnancy if the potential benefit to the mother clearly outweighs any possible risk to the fetus, and only under the guidance of a specialist familiar with the patient’s liver condition. Pregnant women should not self-initiate this medicine. If you are pregnant or planning to become pregnant, inform your doctor so that the risk–benefit balance can be carefully assessed and alternative options considered if appropriate.
There is insufficient information about the excretion of L-ornithine L-aspartate into human breast milk. Because of the limited safety data, Hepamerz syrup should be used during breastfeeding only if clearly needed and prescribed by a physician who has evaluated the clinical situation. The doctor may consider the severity of the mother’s liver disease, the expected benefit of treatment, and any potential risk to the infant. If you notice any unusual symptoms in the breastfed child, inform the pediatrician immediately.
Clinically significant drug–drug interactions with L-ornithine L-aspartate are not widely reported, but comprehensive interaction data are limited. Because Hepamerz syrup is used in patients who often take multiple medicines, including diuretics, lactulose, antibiotics, and drugs for complications of liver disease, it is important to inform your doctor and pharmacist about all prescription medicines, over-the-counter products, and herbal supplements you use. Alcohol should be avoided while taking Hepamerz syrup, especially in patients with liver disease. Alcohol is directly hepatotoxic and can worsen liver damage, increase ammonia production, and counteract the intended benefits of therapy. Continuing to drink alcohol may accelerate progression of liver disease and increase the risk of complications such as hepatic encephalopathy, gastrointestinal bleeding, and fluid accumulation. If you have difficulty stopping alcohol use, discuss this with your doctor so that appropriate support can be arranged.
If you miss a dose of Hepamerz syrup, take it as soon as you remember, unless it is almost time for your next scheduled dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for the missed one, as this may increase the risk of gastrointestinal side effects. If you frequently forget doses, speak with your doctor or pharmacist about strategies to improve adherence, such as reminders or dosing aids.
The onset and extent of clinical improvement with Hepamerz syrup vary depending on the underlying liver condition, baseline ammonia levels, and concurrent treatments. In some patients with hepatic encephalopathy, neurological symptoms may begin to improve over several days when Hepamerz syrup is used alongside standard measures such as lactulose and dietary management. However, it is not a rapid-acting rescue medicine, and it must be used as part of a broader treatment plan. Regular follow-up and laboratory monitoring help the doctor assess response and adjust therapy.
