Most side effects are digestive and are most common when starting or increasing the dose; they usually ease over time. Common: nausea, diarrhoea, reduced appetite, vomiting, constipation, indigestion, and abdominal pain. Injection-site reactions and tiredness can also occur.Others: mild dizziness; if combined with insulin or a sulfonylurea, low blood sugar (hypoglycaemia). Prolonged vomiting/diarrhoea can cause dehydration and affect the kidneys.Serious (seek medical attention): signs of pancreatitis (severe, persistent stomach pain that may spread to the back, with or without vomiting); gallbladder problems (upper-right abdominal pain, fever, yellowing of skin/eyes); severe allergic reactions; and vision changes in people with diabetic eye disease. Tirzepatide carries a boxed warning about a risk of thyroid C-cell tumours seen in animals; it should not be used by people with a personal or family history of medullary thyroid cancer or MEN 2.
Draw up the full single dose from the vial into a syringe and inject it under the skin of your abdomen, thigh, or upper arm once a week, on the same day each week, at any time of day, with or without food. Each vial is single-use — do not reuse or share it. Your doctor or nurse will show you the correct technique and needle before you start.
Not necessarily. After the 2.5 mg starting month, 5 mg is the first maintenance dose. If more blood-sugar control is needed and it is well tolerated, your doctor may step it up further (up to a maximum of 15 mg). Do not change the dose yourself.
Take it as soon as possible if it is within a few days (generally up to 4 days/96 hours) of the missed dose, then continue your regular weekly schedule. If more time has passed, skip it and take the next dose on the usual day. Do not take two doses close together.
Nausea is common early on; eating smaller, bland meals and stopping when full can help. Stay well hydrated, especially if you have vomiting or diarrhoea, to protect your kidneys. Tell your doctor if stomach symptoms are severe or persistent, or if you get severe stomach pain.
Keep it refrigerated (do not freeze) and protected from light. Depending on the product, it may be kept at room temperature for a limited number of days — check the pack insert. Use a new sterile syringe and needle for each dose, dispose of sharps safely in a puncture-proof container, and never reuse or share needles or vials.
