Common: hot flushes and night sweats, headache, uterine bleeding or spotting, hair thinning, mood changes, and reduced sex drive. Others: raised blood pressure and gallbladder problems (gallstones). Serious (seek medical attention): blood clots — signs include pain or swelling in one leg, sudden chest pain or breathlessness, or sudden severe headache, weakness, or vision/speech changes (possible stroke); severe abdominal or right-upper-belly pain (gallbladder); yellowing of the skin or eyes (liver); and new or worsening depression or thoughts of self-harm. Long-term use also causes bone thinning (reduced bone density) that may not fully reverse — which is why treatment duration is limited. Report these promptly.
Take one tablet once a day at about the same time each day, with or without food. It is usually started within the first 7 days of a menstrual period. If you miss a dose, take it as soon as you remember the same day, then continue as usual — do not take two on one day.
Because it can gradually weaken bones, treatment is limited in duration (typically up to about 24 months). Your doctor may arrange bone-density scans and recommend calcium, vitamin D, and weight-bearing exercise to protect your bones. Follow their guidance closely.
It is not a reliable contraceptive and must not be used in pregnancy — it can cause pregnancy loss. If you can become pregnant, use effective non-hormonal contraception (such as condoms) during treatment and for at least one week after stopping. Tell your doctor immediately if you think you may be pregnant.
It should be avoided by women with a history of blood clots, certain cancers, unexplained vaginal bleeding, significant liver disease, or high stroke/clot risk. The risk of clots is higher if you are over 35 and smoke, so smoking should be avoided. You should also not drink alcohol while on it. Tell your doctor your full medical history and all medicines you take.
Contact your doctor promptly about leg pain/swelling, chest pain, breathlessness, severe headache, vision changes, severe abdominal pain, yellowing of the skin/eyes, or any mood changes or thoughts of self-harm. Keep all follow-up and bone-scan appointments.
