Rumanil is a combination analgesic and anti-inflammatory medicine containing diclofenac (sustained release) and paracetamol. It is used for the short-term management of pain and inflammation in conditions such as osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, low back pain, musculoskeletal injuries, sprains and strains, and other painful conditions where the prescriber considers a diclofenac–paracetamol combination appropriate.
Rumanil works through two active components. Diclofenac is a nonsteroidal anti-inflammatory drug (NSAID) that inhibits cyclo-oxygenase (COX) enzymes, reducing the production of prostaglandins responsible for pain, inflammation, and fever. The sustained-release form maintains more stable diclofenac levels over time. Paracetamol acts mainly in the central nervous system to increase the pain threshold and reduce fever. Together, they provide additive pain relief and help control inflammatory symptoms.
You should not decide your own dose of Rumanil. The appropriate dose, dosing frequency, and duration of treatment must be determined by a qualified healthcare professional based on your age, weight, kidney and liver function, coexisting medical conditions, and concurrent medications. Exceeding the prescribed dose or taking it more frequently than advised increases the risk of serious side effects, including gastrointestinal bleeding, kidney damage, cardiovascular events, and liver injury from paracetamol.
Rumanil is generally intended for short-term use at the lowest effective dose. The exact duration should be decided by your doctor according to the underlying condition and your response to treatment. Prolonged continuous use of diclofenac-containing products is associated with increased risks of gastrointestinal ulcers or bleeding, kidney impairment, and cardiovascular events, while long-term or high-dose paracetamol use can cause liver damage. Do not continue Rumanil beyond the period recommended by your prescriber without medical review.
Commonly reported side effects with diclofenac–paracetamol combinations include nausea, vomiting, abdominal pain, indigestion, heartburn, flatulence, diarrhea or constipation, headache, dizziness, and mild elevation of liver enzymes. Some people may experience fluid retention, mild increase in blood pressure, or skin rash. These effects are usually dose-related. If side effects are persistent, troublesome, or worsening, you should contact your healthcare provider for assessment and possible adjustment or discontinuation of therapy.
Serious adverse effects that require urgent medical attention include signs of gastrointestinal bleeding or ulceration (such as black, tarry stools, blood in stools, or vomiting blood), severe or persistent abdominal pain, chest pain, sudden shortness of breath, weakness on one side of the body, slurred speech, severe headache, swelling of the face, lips, tongue, or throat, difficulty breathing, severe skin reactions (blistering, peeling, widespread rash), yellowing of the skin or eyes, dark urine, unexplained fatigue, or marked reduction in urine output. These may indicate gastrointestinal, cardiovascular, allergic, hepatic, or renal complications.
You should not combine Rumanil with other NSAIDs (such as ibuprofen, naproxen, or additional diclofenac) unless specifically instructed by your doctor, because this increases the risk of gastrointestinal bleeding, kidney damage, and other adverse effects. You should also avoid taking additional paracetamol-containing products concurrently, as this can lead to accidental paracetamol overdose and serious liver injury. If your current pain control is inadequate, consult your healthcare provider rather than adding other analgesics on your own.
Diclofenac, like other NSAIDs, can increase the risk of cardiovascular events such as heart attack and stroke, particularly when used at higher doses or for prolonged periods. It may also cause fluid retention and can worsen blood pressure control in some patients. Individuals with existing heart disease, history of stroke, peripheral arterial disease, or uncontrolled hypertension should use Rumanil only under close medical supervision, if at all. Your doctor will weigh the potential benefits against the cardiovascular risks and may choose alternative therapies if appropriate.
Before starting Rumanil, inform your doctor about any history of peptic ulcers, gastrointestinal bleeding, inflammatory bowel disease, liver disease, kidney disease, heart failure, ischemic heart disease, stroke or transient ischemic attack, high blood pressure, asthma or other respiratory allergies, coagulation disorders, or chronic alcohol use. Provide a complete list of all prescription medicines, over-the-counter drugs, herbal products, and supplements you are taking, especially anticoagulants, antiplatelet agents, other NSAIDs, corticosteroids, diuretics, antihypertensives, certain antidepressants, antiepileptics, and any other paracetamol-containing products. This information helps your doctor assess suitability, adjust therapy, and minimize interaction risks.
If you miss a scheduled dose of Rumanil, take it as soon as you remember, unless it is almost time for your next dose. If the next dose is near, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for the missed one, as this may increase the risk of adverse effects. If you frequently forget doses or are unsure how to adjust your schedule, consult your healthcare provider or pharmacist for individualized advice.
Use of fixed-dose diclofenac–paracetamol combinations like Rumanil in children depends on age, body weight, and local regulatory approvals, and should only occur under the guidance of a pediatrician or appropriately trained clinician. Safety and dosing recommendations may differ from adults, and the risk of adverse effects, particularly gastrointestinal, renal, and hepatic toxicity, must be carefully considered. Do not give Rumanil to a child without explicit medical advice and a clear, individualized dosing plan from a healthcare professional.
