Digestive side effects are the most common, especially when starting or increasing the dose, and usually ease within a few weeks. These include nausea, vomiting, diarrhoea, constipation, abdominal pain, and reduced appetite. Less common: burping, indigestion, tiredness, and dizziness. If SEMALIX is combined with insulin or a sulfonylurea, blood sugar can drop too low (hypoglycaemia). Serious (seek medical attention): severe or persistent stomach pain that may spread to the back (possible pancreatitis), signs of gallbladder problems, dehydration from prolonged vomiting/diarrhoea, or allergic reactions. Semaglutide carries a boxed warning regarding a risk of thyroid C-cell tumours seen in animal studies; it should not be used by people with a personal or family history of medullary thyroid cancer or MEN 2.
Take one tablet once a day, first thing in the morning on an empty stomach, with a small sip of plain water (no more than about 120 ml / 4 oz). Swallow it whole — do not split, crush, or chew it. Then wait at least 30 minutes before eating, drinking anything else, or taking other medicines.
The 3 mg dose is a starting dose meant to help your body adjust and reduce side effects. It is normally taken for about 4 weeks, after which your doctor steps you up to a higher dose that actually controls blood sugar. Do not stay on 3 mg long-term unless your doctor advises it.
Skip the missed dose and take your next tablet at the usual time the following morning. Do not take two tablets on the same day to make up for a missed one.
