Can add to the glucose-lowering effect of antidiabetic medicines — monitoring may be needed; Not a substitute for glycaemic control in diabetic neuropathy; Much more potent than plain vitamin D and far more likely to cause hypercalcaemia — calcium and phosphate must be monitored; Needs stomach acid to absorb — take with food; less suitable with PPIs or achlorhydria (calcium citrate is preferred); Evidence for glucose or weight benefit is weak and inconsistent; Evidence varies by indication and is not a substitute for prescribed treatment; B12 deficiency can mask or coexist with folate deficiency — both should be assessed; Prolonged high-dose use can itself cause peripheral neuropathy; and Folic acid can correct the anaemia of vitamin B12 deficiency while allowing neurological damage to progress — B12 status should be checked.
