It should be used with extreme caution, if at all, in patients with severe liver or kidney impairment, uncontrolled epilepsy, head injury, increased intracranial pressure, or a history of substance use disorder. Alcohol should be avoided while taking Tapal ER 50. Both alcohol and tapentadol depress the central nervous system, and their combined use can significantly increase the risk of severe drowsiness, respiratory depression, low blood pressure, impaired judgment, and potentially fatal overdose. If you consume alcohol regularly, inform your doctor before starting Tapal ER 50 so that risks can be assessed and monitored.
If you miss a dose of Tapal ER 50, take it as soon as you remember, unless it is almost time for your next scheduled dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not take two doses at the same time or take extra tablets to make up for a missed dose, as this may increase the risk of side effects or overdose. Tapal ER 50 is generally not recommended during pregnancy unless the potential benefit justifies the potential risk to the fetus. Prolonged use during pregnancy may lead to neonatal opioid withdrawal syndrome in the newborn. During labor, opioid use may cause respiratory depression in the newborn. Tapentadol passes into breast milk, and breastfeeding while taking Tapal ER 50 may expose the infant to opioid effects, including excessive sleepiness and breathing problems. Decisions about use in pregnancy or breastfeeding must be made by a physician after careful risk–benefit assessment.
