Common: tiredness, dizziness or lightheadedness, headache, a slow heart rate (bradycardia), and cold hands and feet. Some people notice low blood pressure symptoms, especially on standing up. Others: shortness of breath, sleep disturbance or vivid dreams, low mood, and mildly raised potassium (from the telmisartan component). Serious (seek medical attention): a very slow or irregular heartbeat, fainting, marked breathlessness or wheezing (metoprolol can worsen asthma/COPD), swelling of the legs or worsening heart failure, or signs of an allergic reaction (swelling of face/lips, difficulty breathing). Report these promptly.
Take one tablet once a day, at about the same time each day, with or after food. Swallow it whole (or, if scored, as directed) with water — do not crush or chew it, as this affects the slow release of metoprolol.
No — this is very important. Stopping a beta-blocker like metoprolol abruptly can cause a dangerous rebound: a racing heart, rise in blood pressure, chest pain, or even a heart attack. If it needs to be stopped, your doctor will lower the dose gradually over 1–2 weeks. Never stop on your own.
If your pulse feels very slow (below about 50 beats a minute) or you feel faint, unusually breathless, or your ankles swell, contact your doctor. Stand up slowly to avoid dizziness, especially when starting treatment.
No. Telmisartan can seriously harm an unborn baby, especially in the second and third trimesters. Tell your doctor immediately if you are pregnant, planning pregnancy, or breastfeeding so a safer alternative can be arranged.
Tell your doctor if you have asthma/COPD, a slow heart rate or certain heart-rhythm problems, diabetes (beta-blockers can mask low-blood-sugar warning signs), or thyroid or circulation problems. Avoid potassium supplements/salt substitutes unless approved, and mention heart-rate-lowering drugs (like diltiazem, verapamil, digoxin) which can add to the slowing effect.
