Common: urinary tract infections and genital fungal infections (from sugar in the urine — more frequent in women), increased urination, headache, dizziness, and cold-like symptoms (nasopharyngitis). Others: mild dehydration or low blood pressure on standing; if combined with insulin or a sulfonylurea, low blood sugar (hypoglycaemia). Serious (seek medical attention): symptoms of diabetic ketoacidosis (nausea, vomiting, stomach pain, fast breathing, unusual tiredness, fruity-smelling breath) even when blood sugar is not very high; a rare but serious genital infection (Fournier's gangrene — severe pain, swelling, redness around the genitals/anus with fever); signs of pancreatitis (severe, persistent stomach pain); signs of liver problems (yellowing of skin/eyes, dark urine, persistent nausea); and severe allergic or skin reactions. Stop the medicine and seek urgent care if these occur.
Take one tablet once a day, at about the same time each day, with or without food. Swallow the sustained-release tablet whole with water — do not crush, split, or chew it, as this affects the slow release of vildagliptin.
Because dapagliflozin removes sugar through the urine, it can encourage yeast and bacteria to grow. Good genital hygiene, staying well hydrated, and drying properly help lower the risk. Tell your doctor if you notice itching, discharge, burning during urination, or pain — these are usually easy to treat when caught early.
You can become dehydrated, which raises the risk of side effects. Drink fluids and contact your doctor for advice — the medicine may be paused temporarily. Seek urgent care for stomach pain, vomiting, fast breathing, or unusual drowsiness.
On its own the risk is low. It rises if you also take insulin or a sulfonylurea. Learn the signs — shakiness, sweating, confusion — and keep a fast sugar source handy.
