Ascabiol is a topical medicated lotion that contains cetrimide 0.1% w/v and gamma benzene hexachloride (lindane) 1% w/v. It is primarily used for the treatment of scabies, a contagious skin infestation caused by the mite Sarcoptes scabiei. In some settings, it may also be used for certain lice infestations, depending on local prescribing information. The lindane component kills mites and lice, while cetrimide provides antiseptic action that helps reduce bacterial contamination of the skin.
Ascabiol works mainly through its active ingredient lindane, an organochlorine scabicide. Lindane is absorbed through the exoskeleton of the scabies mite and interferes with its nervous system, leading to hyperexcitation, paralysis, and eventual death of the parasite. Cetrimide, the other active component, is a cationic surfactant with antiseptic properties that helps reduce the risk of secondary bacterial infection in the affected skin areas. Together, these actions help clear the infestation when the product is used correctly.
Ascabiol is for external use only and should be applied exactly as directed by a healthcare professional and according to the product leaflet. Typically, it is applied to clean, dry, unbroken skin, covering the entire body surface from the neck down, and in some cases including the scalp and face (avoiding eyes, mouth, and mucous membranes) if specifically advised. The exact amount to use, how long it should remain on the skin, and whether a repeat application is needed vary by guideline and patient factors, so you should not guess or self-prescribe. Do not apply to open wounds, severely inflamed skin, or large areas in infants or small children without medical supervision.
Ascabiol should not be applied to the eyes, eyelids, or mucous membranes, and contact with the mouth, nose, and genital openings must be avoided. In some cases of severe or crusted scabies, a clinician may advise cautious application to parts of the face or scalp, but this must be done strictly under medical supervision with clear instructions. If the lotion accidentally gets into the eyes or mouth, rinse immediately with plenty of clean water and seek medical advice if irritation or symptoms persist.
Lindane-containing products such as Ascabiol have a higher risk of systemic absorption and neurotoxicity in infants and young children because of their larger skin surface area relative to body weight and more permeable skin. Many guidelines recommend avoiding lindane in premature infants, neonates, and young children, or using it only when no safer alternatives are available and under strict specialist supervision. If a child has been prescribed Ascabiol, caregivers must follow the exact instructions regarding the amount, application area, and contact time, and must prevent the child from licking or ingesting the product.
Common local side effects of Ascabiol include temporary burning, stinging, itching, dryness, or redness at the application site. These reactions are usually mild and resolve on their own. Less commonly, individuals may develop contact dermatitis, blistering, or swelling, which can indicate irritation or allergy. Systemic side effects are rare when the product is used correctly but may include neurological symptoms such as dizziness, headache, restlessness, or, in severe cases, seizures, particularly if excessive amounts are applied, if the skin is damaged, or if the product is used in high-risk patients. Any unusual or severe symptoms require immediate medical attention.
Use of lindane-containing products like Ascabiol during pregnancy is generally approached with caution because lindane can be absorbed through the skin and there are concerns about potential toxicity to the mother and fetus. In many treatment guidelines, lindane is not considered a first-line option for pregnant women and is reserved for situations where safer alternatives are unsuitable or ineffective, and where the expected benefit clearly outweighs the potential risk. A pregnant patient should only use Ascabiol if specifically prescribed by a physician who has assessed her individual clinical situation.
Lindane can be absorbed systemically and has been detected in breast milk. For this reason, the use of Ascabiol in breastfeeding women is generally restricted and considered only when clearly necessary and when alternative treatments are not appropriate. If a clinician decides that Ascabiol is required, they may advise precautions such as temporarily interrupting breastfeeding, avoiding application to the breast area, and ensuring that the infant does not come into direct contact with treated skin until the product has been thoroughly washed off. Breastfeeding mothers should discuss risks and benefits with their healthcare provider before use.
Scabies is highly contagious and spreads through close skin-to-skin contact and, less commonly, through shared clothing or bedding. Household members and close physical contacts are often advised to be evaluated and treated simultaneously, even if they do not yet show symptoms, to prevent reinfestation. Clothing, towels, and bedding used in the days before treatment should typically be washed in hot water and dried at a high temperature where possible, or sealed in a plastic bag for a recommended period according to local guidance. These environmental measures should be followed alongside the correct use of Ascabiol as directed by your clinician.
If you have applied more Ascabiol than recommended or left it on for longer than instructed, wash the skin thoroughly with soap and water as soon as you realize the error. Monitor for any signs of irritation, rash, dizziness, confusion, muscle twitching, or seizures. Because lindane has a narrow safety margin, especially in children, elderly individuals, and people with neurological conditions, you should contact a healthcare professional or local poison control center promptly for advice, even if you feel well. Do not reapply the product unless specifically told to do so by a clinician.
Ascabiol is applied topically, and systemic drug–drug interactions are less common than with oral medicines. However, factors that increase skin permeability or alter the barrier function, such as other topical medications, keratolytics, corticosteroids, or harsh cleansers, may increase the absorption of lindane and the risk of toxicity. You should inform your healthcare provider about all other topical and systemic medicines you are using, including over-the-counter products and herbal preparations. Avoid applying other products on the same skin areas at the same time unless your clinician has advised a specific regimen.
