The most important effect is low blood sugar (hypoglycaemia) from glimepiride — shakiness, sweating, hunger, palpitations, dizziness, confusion. From metformin: nausea, diarrhoea, metallic taste, and (long-term) low vitamin B12. Serious (seek medical attention): severe or prolonged low blood sugar; lactic acidosis (unusual tiredness, muscle aches, trouble breathing, feeling very cold, stomach pain); or allergic reactions.
Take it with breakfast or your first main meal (as prescribed), swallowing the tablet whole — do not crush the sustained-release part. Do not skip meals after taking it, to avoid low blood sugar.
Take a fast-acting sugar (glucose tablets, juice, or sugar), then a snack or meal. Always carry a sugar source and learn the warning signs. A severe episode needs emergency care.
If dehydrated or scheduled for surgery or a contrast scan, tell your doctor — metformin may be paused temporarily to reduce the risk of lactic acidosis.
Yes. You should not drink alcohol, as it can cause severe low blood sugar with glimepiride and raises the risk of lactic acidosis with metformin.
Yes. Your doctor monitors blood sugar (HbA1c), kidney and sometimes liver function, and vitamin B12. Tell your doctor if you are pregnant or breastfeeding.
