Most side effects are mild and often settle as the body adjusts. Common: dizziness or lightheadedness (especially on standing up), headache, and increased urination, particularly early in treatment. Because hydrochlorothiazide is a diuretic, it can affect body salts and hydration — watch for excessive thirst, dry mouth, muscle cramps or weakness (signs of altered sodium or potassium). It may also raise uric acid (which can trigger gout), blood sugar, and cholesterol, and increase sensitivity to sunlight. Note: telmisartan tends to raise potassium while hydrochlorothiazide tends to lower it, so your doctor monitors electrolytes to keep them balanced. Serious (seek medical attention): fainting, very low blood pressure, irregular heartbeat, marked reduction in urine output, severe skin reactions, or allergic reactions (swelling of face/lips, difficulty breathing).
Take one tablet once a day, with or without food, preferably at the same time each morning. Swallow it whole with water. Taking it in the morning reduces night-time trips to the bathroom from the diuretic effect.

No. Medicines like telmisartan can seriously harm an unborn baby, especially in the second and third trimesters. Tell your doctor immediately if you are pregnant, planning pregnancy, or breastfeeding so a safer alternative can be arranged.
Avoid potassium-containing salt substitutes and potassium supplements unless your doctor approves, and limit alcohol as it can lower blood pressure too much. Stay well hydrated, use sun protection, and tell your doctor if you have severe vomiting or diarrhoea, as dehydration can cause your blood pressure to drop sharply.
Yes. Your doctor will check your blood pressure and periodically test kidney function, salts (sodium, potassium), uric acid, blood sugar, and cholesterol to make sure the medicine is working safely. Keep up with these appointments.
