Most side effects are digestive and linked to metformin, and usually ease with time; taking the tablet with food helps. Common: nausea, vomiting, diarrhoea, abdominal pain, indigestion, loss of appetite, and a metallic taste. Others: headache, dizziness, and — rarely — mild changes in liver enzymes. Long-term metformin use can lower vitamin B12 levels. Serious (seek medical attention): lactic acidosis — a rare but serious build-up of acid in the blood; warning signs include unusual tiredness, muscle aches, difficulty breathing, feeling very cold, stomach pain, and slow or irregular heartbeat. Also watch for signs of liver problems (yellowing of skin/eyes, dark urine, persistent nausea, severe tiredness), pancreatitis (severe, persistent stomach pain spreading to the back), and severe allergic or skin reactions. Stop the medicine and seek urgent care if these occur.
Take it twice a day with meals (usually one with breakfast and one with the evening meal), or exactly as your doctor directs. Swallow the tablet whole with water. Taking it with food reduces stomach upset.
Vildagliptin can rarely affect the liver, so your doctor usually checks liver function before starting and periodically during treatment. Report any yellowing of the skin or eyes, dark urine, or persistent nausea and tiredness promptly.
If you become dehydrated (vomiting, diarrhoea, poor fluid intake) or are scheduled for surgery or an X-ray/scan using contrast dye, tell your doctor — metformin may need to be paused temporarily to reduce the risk of lactic acidosis. Do not stop or restart on your own without advice.
On its own the risk is low. It rises if you also take insulin or a sulfonylurea, skip meals, exercise heavily, or drink alcohol. Learn the signs — shakiness, sweating, confusion — and keep a fast sugar source handy.
Yes. Your doctor will monitor blood sugar (including HbA1c), kidney function, and liver function periodically, and may check vitamin B12 with long-term use. Keep up with follow-up appointments and blood tests, and limit alcohol.
