Most people tolerate short courses well. Common side effects include nausea, vomiting, diarrhoea, and headache. Less common: altered taste, changes in blood counts (especially low platelets — thrombocytopenia), and mild changes in liver enzymes. Blood counts should be monitored, particularly with courses longer than 2 weeks. Serious (seek medical attention): severe or persistent diarrhoea (possible C. difficile infection); nerve problems such as numbness, tingling, or vision changes (peripheral or optic neuropathy), reported mainly with use beyond 28 days; serotonin syndrome (agitation, confusion, fast heartbeat, sweating) if combined with certain antidepressants; and, rarely, lactic acidosis (unusual tiredness, fast breathing).
Take one tablet by mouth every 12 hours (twice a day), with or without food, exactly as prescribed. Take it at evenly spaced, fixed times each day and complete the full course even if you feel better.
Yes. Linezolid also has mild MAO-inhibitor activity, so avoid large amounts of tyramine-rich foods — aged cheeses, cured/fermented meats (salami), pickled or smoked fish, soy sauce, tofu, sauerkraut, tap/draught beer, and red wine. These can cause a sudden rise in blood pressure. Limit caffeine as well.
Tell your doctor about all your medications, especially antidepressants (SSRIs, SNRIs, MAOIs), migraine medicines, and decongestants, as combining them with linezolid can be dangerous. Do not start any new medicine without checking with your doctor.
Courses typically run 10–14 days depending on the infection. Do not stop early or extend it on your own — treatment beyond 28 days raises the risk of nerve and blood-related side effects and needs close medical supervision.
It's best to avoid alcohol, especially tap beers and red wine, which are high in tyramine and can interact with linezolid to raise blood pressure. Alcohol can also worsen nausea.
