Common, often settling with time: nausea, diarrhoea, abdominal discomfort, a metallic taste, and reduced appetite; taking it with food helps. Long-term use can lower vitamin B12. Serious (rare, seek medical attention): lactic acidosis — unusual tiredness, muscle aches, difficulty breathing, feeling very cold, stomach pain, and a slow or irregular heartbeat. Stop and seek urgent care if these occur.
Take it once a day with the evening meal (or as directed), swallowing the sustained-release tablet whole — do not crush or chew it. Taking it with food reduces stomach upset.
If you are dehydrated (vomiting, diarrhoea, poor fluid intake) or scheduled for surgery or a scan using contrast dye, tell your doctor — metformin may be paused temporarily to reduce the risk of lactic acidosis.
On its own the risk is low. It rises if combined with insulin or a sulfonylurea, or with missed meals or alcohol. Do not drink alcohol, which also raises the risk of lactic acidosis.
Long-term use can lower vitamin B12, so your doctor may check it periodically and treat if needed.
