• The two medicines act on different targets. Dapagliflozin is an SGLT2 inhibitor: it blocks the reabsorption of glucose in the kidneys, so extra sugar is passed out in the urine, lowering blood sugar. Linagliptin is a DPP-4 inhibitor: it raises the levels of natural gut hormones (incretins like GLP-1 and GIP), which prompt the pancreas to release more insulin and reduce glucagon after meals. Working together, they lower blood sugar in a glucose-dependent way, which keeps the risk of low blood sugar relatively low when used alone.

Common: urinary tract infections and genital fungal infections (due to sugar in the urine — more frequent in women), increased urination, and cold-like symptoms (nasopharyngitis). Some people feel dizzy or lightheaded, especially if dehydrated. Others: mild dehydration or low blood pressure on standing, and — if combined with insulin or a sulfonylurea — low blood sugar (hypoglycaemia). Serious (seek medical attention): symptoms of diabetic ketoacidosis (nausea, vomiting, stomach pain, fast breathing, unusual tiredness, fruity-smelling breath) even when blood sugar is not very high; a rare but serious genital infection (Fournier's gangrene — severe pain, swelling, redness around the genitals/anus with fever); signs of pancreatitis (severe abdominal pain); joint pain; or allergic reactions.
