Justoza SM 500 is used for the management of type 2 diabetes mellitus in adults. It helps lower blood glucose levels when diet, exercise, and other antidiabetic medicines alone do not provide adequate control. It is not intended for type 1 diabetes or diabetic ketoacidosis.
Justoza SM 500 combines three antidiabetic agents with different mechanisms. Dapagliflozin (an SGLT2 inhibitor) increases urinary glucose excretion by blocking glucose reabsorption in the kidneys. Metformin (a biguanide, sustained-release) reduces glucose production in the liver and improves insulin sensitivity. Sitagliptin (a DPP-4 inhibitor) increases incretin hormone levels, enhancing glucose-dependent insulin release and reducing glucagon secretion. Together, they improve fasting and post-meal blood glucose control.
When used alone, the components of Justoza SM 500 have a relatively low intrinsic risk of hypoglycaemia because metformin does not stimulate insulin secretion and sitagliptin acts in a glucose-dependent manner. However, the risk of low blood sugar may increase if it is taken together with other medicines that can cause hypoglycaemia, such as sulfonylureas or insulin, or if meals are skipped, caloric intake is reduced excessively, or there is unaccustomed strenuous exercise. Symptoms such as sweating, tremor, palpitations, hunger, or confusion should be reported promptly.
Commonly reported side effects with the components of Justoza SM 500 may include urinary tract infections, genital fungal infections, increased urination, nausea, diarrhoea, abdominal discomfort, indigestion, headache, and mild upper respiratory symptoms. Many gastrointestinal effects are related to metformin and may improve over time. Genital and urinary infections are associated with dapagliflozin due to increased glucose in the urine. Any persistent, severe, or unusual symptoms should be discussed with a healthcare professional.
Use of Justoza SM 500 in patients with kidney impairment requires careful assessment. Metformin is contraindicated or requires dose adjustment in moderate to severe renal dysfunction due to the risk of lactic acidosis. Dapagliflozin efficacy decreases with reduced kidney function and may be contraindicated below certain estimated glomerular filtration rate (eGFR) thresholds. Sitagliptin also requires dose adjustment in renal impairment. Kidney function must be evaluated before starting therapy and monitored regularly, and the prescribing clinician will decide whether this combination is appropriate.
Justoza SM 500 is generally not recommended during pregnancy because adequate, well-controlled data on the combined use of dapagliflozin, metformin, and sitagliptin in pregnant women are lacking, and potential risks to the developing fetus cannot be excluded. Insulin-based regimens are usually preferred for blood glucose control in pregnancy. Women who are pregnant, planning pregnancy, or suspect they may be pregnant should consult their doctor promptly for an alternative treatment plan.
Information on the excretion of dapagliflozin and sitagliptin into human breast milk is limited, and metformin is known to pass into breast milk in small amounts. Because of the potential risk to the nursing infant, the use of Justoza SM 500 during breastfeeding is generally not advised unless the expected benefit to the mother clearly outweighs any possible risk to the child. A healthcare professional should evaluate the need for treatment and may recommend either an alternative medicine or discontinuation of breastfeeding.
The exact dosing schedule, including the number of tablets and timing, must be prescribed by a registered healthcare professional based on individual clinical factors. It is usually taken orally once or more times daily with food or as directed, to help reduce gastrointestinal side effects related to metformin. Tablets should be swallowed whole with water and not crushed or chewed, especially because the metformin component is sustained-release. Patients should follow the prescribed regimen strictly and not adjust the dose on their own.
Before starting Justoza SM 500, inform your doctor about any kidney or liver disease, heart failure, history of pancreatitis, recurrent urinary or genital infections, dehydration, alcohol misuse, or planned surgery or imaging procedures involving iodinated contrast. Maintain adequate hydration, monitor blood glucose as advised, and report symptoms such as severe abdominal pain, persistent vomiting, rapid breathing, unusual fatigue, reduced urine output, or signs of infection. Do not use this medicine for type 1 diabetes or diabetic ketoacidosis.
Regular or excessive alcohol intake is not advisable while taking Justoza SM 500. Alcohol can increase the risk of metformin-associated lactic acidosis, particularly in the presence of fasting, malnutrition, liver disease, or acute illness. It may also affect blood glucose control and increase the risk of hypoglycaemia or hyperglycaemia. Occasional small amounts may be permissible for some patients, but this should be discussed with the treating physician, who can provide individualized guidance.
If a dose of Justoza SM 500 is missed, it should generally be taken as soon as the patient remembers, unless it is almost time for the next scheduled dose. In that case, the missed dose should be skipped and the regular dosing schedule resumed. Two doses should not be taken at the same time to make up for a missed dose, as this may increase the risk of side effects. Persistent problems with remembering doses should be discussed with a healthcare professional.
