Most side effects are digestive and related to metformin, and often ease as the body adjusts; the SR form and taking it with food help reduce them. Common: nausea, diarrhoea, flatulence, abdominal discomfort, indigestion, and a metallic taste. Headache, cold-like symptoms (nasopharyngitis), and mild upper respiratory or urinary infections can also occur.Others: reduced appetite; long-term metformin use can lower vitamin B12 levels.Serious (seek medical attention): lactic acidosis — a rare but serious build-up of acid in the blood; warning signs include unusual tiredness, muscle aches, trouble breathing, feeling very cold, stomach pain, and slow or irregular heartbeat. Also watch for signs of pancreatitis (severe, persistent stomach pain, often spreading to the back), severe allergic reactions, or joint pain. Stop the medicine and seek urgent care if these occur.
Take it once daily with a meal (usually the evening meal) or exactly as your doctor directs. Swallow the sustained-release tablet whole with water — do not crush, split, or chew it, as this can affect the slow release of metformin. Taking it with food reduces stomach upset.
If you are dehydrated (vomiting, diarrhoea, not drinking enough) or scheduled for surgery or an X-ray/scan using contrast dye, tell your doctor — metformin may need to be paused temporarily to lower the risk of lactic acidosis. Do not stop or restart on your own without advice.
On its own, the risk is low. It increases if you also take insulin or a sulfonylurea, skip meals, exercise heavily, or drink alcohol. Learn the signs — shakiness, sweating, confusion — and keep a fast sugar source handy.
Limit or avoid alcohol. It can lower blood sugar and raises the risk of lactic acidosis with metformin. If you do drink, do so in moderation and not on an empty stomach, and discuss it with your doctor.
Yes. Your doctor will monitor blood sugar (including HbA1c) and kidney function periodically, and may check vitamin B12 with long-term use. Keep up with your follow-up appointments and blood tests.
