Most side effects are mild and often settle as the body adjusts. Others: tiredness, back pain, diarrhoea, nausea, and mild upper respiratory infection. Because chlorthalidone is a diuretic, it can affect body salts and hydration — watch for excessive thirst, dry mouth, muscle cramps or weakness. It may also raise uric acid (which can trigger gout) and blood sugar. Note: telmisartan tends to raise potassium while chlorthalidone tends to lower it, so your doctor monitors electrolytes to keep them balanced. Serious (seek medical attention): fainting, a very low blood pressure, irregular heartbeat, signs of an allergic reaction (swelling of face/lips, difficulty breathing), or a marked drop in urine output.
Take one tablet once a day, with or without food, preferably at the same time each morning. Swallow it whole with water. Taking it in the morning reduces night-time trips to the bathroom from the diuretic effect.
